Full tooth extraction and restoration

Elena came to our clinic, unfortunately, in a condition where all her teeth were already completely destroyed.
At that point, it was no longer possible to restore them or place bridge prostheses supported by the existing teeth.

In such a situation, there was really only one solution — full tooth extraction followed by implant placement.
The “All-on-4” technique was used in combination with the placement of two pterygoid implants in the posterior sections of the jaw.
This approach is effective because it allows for even distribution of the load across all teeth.

At the same time, this implant configuration makes it possible to bypass the maxillary sinus at an angle and anchor the implants in the pterygoid processes of the sphenoid bone — bony structures that provide stable fixation for the implants.
Another advantage of this method is that it eliminates the need for bone grafting or sinus lifting, and therefore avoids the additional risks associated with those procedures.
Reconstructed bone has its own drawbacks — for example, it is more prone to inflammation and may develop peri-implantitis (inflammation around the implant).

This is due to the fact that in cases of long-term tooth loss, the bone tissue in the area receives poor blood supply, and the grafted material may not integrate properly.
An added benefit is that the patient doesn’t need to invest extra resources into these procedures.

That’s why the chosen method is considered a kind of “gold standard.”
For the lower jaw, the All-on-4 technique was also used, combined with two short implants placed in the posterior region near the second molars.
On the day of surgery, the prostheses were fixed in place.
The “after” photos were taken 10 days later — and the transformation is striking. The patient’s appearance had changed so much, it was as if she had become younger.

In fact, the facial recognition system at her workplace didn’t recognize her when she returned — she had to re-register from scratch.
Hopefully, it wasn’t just the soulless machine that appreciated Elena’s new smile, but also her family, friends, and colleagues.
